Accessible Forms

The following was done for Lab 5 in the CMWEB 150 class.

In this lab, I made the form accessible by adding labels, which tied the displayed name of each text field, radio box and text area to it's appropriate form element. With the help of the Lynda.com tutorials, I then used CSS to format the form to be visually appleaing and added a required asterisk. This took quite a bit of time to do, but I know now the importance of accessibility with forms. My forms in the past haven't been accessible and although this form could be improved, the accessibility features that were added greatly help.
